Sixty (60) days prior to your arrival date, we will send you a reminder regarding required forms you’ll need to complete before the trainee attends Camp. These forms must be submitted no later than 30 days prior to arrival in order to fully participate in Camp activities.

 

Please select your program of attendance in order to complete the required forms. Those listed as optional are not required. If there is additional medical information on your child we need to know please complete that form. Please note that our nursing staff may ask for extra information from you regarding certain medical conditions.
